To make things easier for you we have provided our forms on our website. Simply click on the names of the forms you need, print them, fill them out, and just bring the forms with you to your appointment.
In order to provide you and others with excellent customer service and access to appointments we have a firm cancellation policy. If you need to cancel your appointment, please call us at (306) 569-6683 as soon as possible and preferably with 24 hour notice. Since we turn away other clients to hold your appointment, any cancellations with less than 6 hours notice (or no-shows) will result in a $25.00 charge on your credit card. If your visits are funded by a third party payer (e.g. SGI, WCB, Regina Qu’Appelle Health Region), you will be responsible for the cancellation fee.
Arriving more than 15 minutes late for an appointment does not allow enough time to provide an adequate treatment and if the therapist cannot accommodate for the late arrival, the appointment will need to be rescheduled and the cancellation fee will be applied.
More than two missed appointments without notification will result in cancellation of any further pending appointments. You will need to contact the clinic to make additional appointments.
We offer appointment reminders by email 24 hours in advance. Kindly provide us with an email address that you check on a regular basis on the client information form. Remember that although our automatic reservation system does send out reminder emails, email in general is not always reliable. We recommend that clients also note the date/time of their appointments on a Synergy Physio appointment card or in their personal electronic device.
SGI and WCB funded patients should be aware that our clinic is required by law to report non-attendance to the respective funding agency.
